Touchstone Communities is actively seeking a dedicated Licensed Social Worker to join our team, enriching the lives of elders with personalized care in our healthcare environments across Texas. This role is pivotal in designing social programs and providing individualized psycho-social support, ensuring a dignified quality of life for our residents. The ideal candidate will possess a deep understanding of the emotional and social factors influencing elder patients and be committed to fostering an atmosphere of wellness and support. Assess resident needs and create detailed care plans. Facilitate communication between residents, family members, and medical staff. Guide residents and their families through healthcare processes and decisions. Ensure compliance with federal, state, and local regulations. Provide crisis management services when needed. Organize and lead community support groups. Strong interpersonal and communication skills Ability to handle multiple tasks simultaneously Crisis intervention techniques Proficient in patient assessment and family education Strong organizational and time management skills 401k State-specific licensure as a Licensed Social Worker (LSW) is mandatory. CPR certification advised.
